Medical Case Management
Highly trained professionals coordinate and advocate for necessary
services for disabled individuals to maximize their quality
of life. These services are provided to pediatric, adult and
geriatric clients.

Life Care Planning
Lifelong medical need projections and their associated costs
are detailed. These documents are helpful in setting long-term
financial reserves. Rehab Assist professionals are experienced
in deposition and trial testimony.
Elder Care Management
Rehab Assist provides elder care management services to those
elderly individuals who are compromised either mentally or
physically and who may require monitoring, supervision, guidance,
or planning. Services include assessment of needs, creation
of a care plan, life planning, monitoring of care, and budgeting
and coordination of services. Rehab Assist may also coordinate
physician appointments or community activities. A “hands
on” personal approach is utilized at all times.
Guardianship Services
This service monitors and oversees the individual's mental,
physical and emotional status. This is achieved through onsite
monitoring and advocacy for the client. Issues regarding appropriate
residence, medical needs, educational needs and vocational
placement, to name a few, can be addressed. Reports are made
to family members and designated advocates on a regular basis.
Nursing homes, rehab units, school environments and homebound
clients are frequently visited to assure proper care.

Budgets
Often submitted to the court, these documents assist the family,
the disabled individual and the estate planning team to budget
and plan for necessary medical and disability related expenditures.

These documents provide a thorough review of the disabled
individual's status and services they will require.

Educational Resource
Rehab Assist professionals are familiar with Special Education
services and Individualized Educational Plans (IEPs). Rehab
Assist participates in school conferences, coordinating care
with other services and providing guidance to the disabled
individual, their family and the educational team.
Probate Court Testimony
Rehab Assist frequently provides testimony in the probate
courts for both disabled minors and adults. A comprehensive
assessment regarding the individual’s overall well being
is presented to the court. Frequently the court will ask Rehab
Assist to provide recommendations and budgets for necessary
health care services.
